Product Description: Pays a Miscellaneous Hospital Expense Benefit (less a $500 deductible) of up to $15,000 and a Surgical Expense Benefit of up to $5,000 on the GSP Standard-15 and on the GSP Select-25 a Miscellaneous Hospital Expense Benefit (less a $500 deductible) of up to $25,000 and a Surgical Expense Benefit of up to $7,500.
Either plan offers a choice of Daily Hospital Room Benefits from $100 to $400. They also provide an Intensive Care Benefit, Administration of Anesthetic Benefit, Ambulance Benefit, In-Hospital Medical Expense Benefit and Outpatient Hopsital Expenses Benefit.

Product Description: After You meet an Annual Individual Deductible which you select, this plan
provides 80% of covered Usual, Customary and Reasonable (UCR)
Hospital/Surgical charges. The CS1 covers incurred Eligible Expenses up to
the UCR charges for health care received based on actual provider location.
With an annual maximum benefit of up to $100,000 per covered family member,
this plan has NO LIFETIME MAXIMUM!

- Tradition Signature Series (Indemnity - California Only)
Product Description: Tradition Signature Series is hospital, medical and surgical indemnity
expense coverage for individuals and families. Choose from four standard
hospital benefit levels: $50,000, $37,500, $25,000, or $12,500, which are
subject to specific daily dollar limits. Each benefit amount provides
additional coverage for surgeon's fees, a percentage of which is available
for assistant surgeon and anesthesiologist fees. There is also an ambulance
benefit, accidental death benefit, and a mammography screening benefit. In
addition, other inpatient-only and outpatient-only benefits are available.
Several optional health benefits are offered. Additional physician benefit,
cancer rider, critical illness rider, and accident rider. Optional life
add-ons are also available: life policy qualification, whole or term life
policy, and a child term life rider.
